MUST READ BEFORE SUBMITTING A MILITARY EXEMPTION

 

  • MARINES & NAVY -YOU WILL WANT TO USE THE "MILITARY EXEMPTION TEMPLATE" ABOVE FOR YOUR EXEMPTION

  • SCHEDULE A MEETING WITH YOUR MILITARY CHAPLAIN. THIS DOCUMENT IS FOR THE CHAPLAIN TO REVIEW, FILL OUT AND SEND IN TO THE COMMANDING OFFICER

  • BEFORE CONTINUING ON PLEASE THOROUGHLY READ THE ABOVE PDF DOCUMENT "MARINE CORPS ORDER 1730.9"

  • COPY THE "MILITARY EXEMPTION TEMPLATE" AND EDIT TO INCLUDE YOUR PERSONAL INFO

  • ARMY  - See pages 44-46 in the above document "ARMY Regulations 600-20" for information regarding religious vaccine exemptions

  • ALL OTHER ARMED FORCES WILL WANT TO FOLLOW THE "EXAMPLE RE LETTER FOR ARMED FORCES" THAT IS ABOVE AND TURN IT IN TO YOUR CHAPLAIN OR COMMANDING OFFICER. 

  • *DO NOT COPY "The reasons for my sincerely held religious belief is as follows:" YOU MUST WRITE YOUR OWN REASONS!

 

 

How to recreate and fill out the info on the "MILITARY EXEMPTION TEMPLATE":

 

  • "FROM" is your military chaplain's name because this will ultimately be submitted by your chaplain to the Commanding Officer

  • "TO" will say [ Commanding Officer of requester ]. If you do not know the Commanding Officer then leave it blank. You may want to add a line for a name next to [Commanding Officer] ___________________________________

  • "Subj" add your name and rank where it says [Requester's Rank and name]

  • "Ref" remains as it is

  • Sections 1-6:

 

 

  1. Fill in Requester's Rank and Name (your name and rank), as well as the date of the interview that you have scheduled with your chaplain

  2. Nature of the request: You are to explain your request to be exempt from vaccines and why. Share your strongly held religious beliefs and why they prevent you from getting vaccinated. You may use information from the FOR-US website to help build your argument. Include as much detail as possible including scripture that pertains to your beliefs. You may also want to read through other religious exemptions either on the FOR-US website or online to help build your argument. 

  3. Basis: You are to explain more in depth what your religion is, religious practices, and share your professional and objective opinions as to why you must avoid being vaccinated. 

  4. Alternate means: Suggest alternate means for why you can still take part in the military without vaccination. This may include good health, natural immunity, willingness to be tested or take other precautions. 

  5. Sincerity: This is to be left blank and filled out by your chaplain upon your scheduled meeting. You may want to add several lines for your chaplain to write on. (Example: Sincerity___________________________)

  6. My contact information: This is your Chaplain's contact info. Leave this area blank or include a line for your chaplain to write on. You may additionally include your own contact info on a separate line below your chaplain's contact info. 

  7. Leave Signature as is. You may also want to add a line for the chaplain to write his signature. (Example: Signature_________________________)

  8. Copy to: Leave as is.
